In order to be eligible for donation consideration, please complete and submit all of the following information to campaign4change@strikeusa.com. 

• Donation Request Form (Please email to request)
• IRS W-9 Form (Please email to request)
• Verification of Non-Profit 501(c)(3) Tax-Exempt Status

Submit requests at least one month in advance from the date needed in order to allow for processing time. 

Please note that completion of the forms does not guarantee that Campaign for Change will be able to fulfill the request.
